Listed below are all of the 2025-26 Program Year-End Reports that you can use to report on all of the wonderful work you did this year for our Veterans.

Mail In Instructions

1. Every Auxiliary should have participated in at least one program.
   
2. You need to mail all the Year-End Program Reports that you fill out to your District President.
3. You can find fillable forms below.  You can type in your answers and print the report.
4. If there are any questions that you need more room to answer, you can write on the back of the printed form.
5. THE REPORTS MUST BE IN YOUR DISTRICT PRESIDENT’S HANDS BY APRIL 1, 2026.
6. Download the Mail In Instruction Sheet.  It includes the address of the District President you need to mail the forms to.

After you have the Year-End Reports done, don't forget to fill out the Achievement Award form for the opportunity to win your Auxiliary an award at convention.  The completed form and any additional information should be to Sr. Vice President Charlene Cobb, no later than May 1st, 2026.
